Register online
Complete your child's registration through our secure patient portal. Takes about 10 minutes and saves 30 at the front desk. This creates your child's chart with us — every other step builds on it.

Verify your insurance and switch your child's PCP to us
Call the member number on the back of your insurance card. Confirm your plan is active, confirm we're in-network, and ask them to list Dr. Isabel Velez as your child's primary care provider. This must be done before the visit or your plan may not cover it.

Request records from your previous pediatrician
Download our records release form, sign it, and send it to your prior office. The immunization record is the piece we need most — without it we may have to repeat vaccines.

What to expect at the first visit
Plan for about 45 minutes. Dr. Velez will review your child's history, complete a full exam, catch up any needed vaccines, and answer your questions — bring a list! You'll leave with your parent welcome folder and a plan for the year ahead.
